OB3 will undertake a rapid review to assess the spread and scale of approved Transformation Fund proposals. the work will inform communications across Welsh Government structures, the Transformation Programme Advisory Board, and will also contribute to shaping the further implementation of the Transformation Fund in future.
The Welsh Government’s £100million Transformation Fund aims to speed up the development (and scaling up) of new models of health and social care, and to demonstrate their value. The initial focus of the Transformation Fund is on models which make early progress on: seamless alignment of health and social care services; local primary and community-based health and social care delivery; and new integrated prevention services and activities.
The review will provide Welsh Government with:
a high-level commentary and reflection on how the approved proposals align with A Healthier Wales, its future vision and the Transformation Fund guidance
suggestions on strengthening delivery and evaluation.
